Job Description:
A company is looking for an LVN Case Manager in Disease
Management to facilitate a team approach for cost-effective
delivery of quality care and services.
Key Responsibilities
Conduct clinical evaluations and assessments to determine member
eligibility for case management
Develop and implement case management plans in collaboration with
members and healthcare professionals
Maintain a member case load for regular outreach and management,
ensuring compliance with service standards
Required Qualifications
Graduation from an accredited Licensed Vocational Nurse program
Current unrestricted LVN license in California
2+ years of recent clinical experience as an LVN/LPN
1+ years of care management, disease management, or discharge
planning experience
Access to a dedicated workspace and high-speed internet
connection
